Psychosocial Rehabilitation

JOB SUMMARY: Develops and implements psychosocial rehabilitation services for individuals with mental illness for the purpose of recovery and community integration.

1. Coordinates intake procedures, completes paperwork and provides orientation to individuals entering programming
2. Develops, coordinates, implements and monitors individual treatment plans and follow up for identified individuals on caseload
3. Facilitates and coordinates community referral and linkage as necessary for individual consumers
4. Maintains case files, updates medical exams, releases, etc. and ensures all reports (accident/illness, termination, discharge, etc.) are completed accurately and in a timely manner, in addition to attendance and benefits reports
5. Assists PSR Coordinator in the monitoring of the Medicaid program, including: monthly billing, file audits, LPHA consultation, completing mental health assessments and ensuring Department of Human Services Rule 132 compliance
6. Assists in the development and implementation of the Drop-In Center at NRI and LCHD, which is off site.
7. Educate, promote and provide support to individuals in making healthy lifestyle choices, as referenced in the DSP health and safety and health and nutrition training curriculums and/or service plans.
8. Identify and utilize effective advocacy strategies to overcome the diverse challenges facing individuals, i.e., human rights, legal, administrative and financial.
9. Provides crisis intervention, behavior management and counseling as needed
10. Responds to medical, mental health and behavioral emergencies as necessary
11. Conducts and/or assists with psychiatric rehabilitation classes
12. Assists with development of community resources/community integration
13. Develops and implements the Workers Council
14. Develop and implement PSR Volunteers program
15. Attends and participates in Departmental meetings
16. The employee may be asked to perform other duties as required by the employer’s business needs.

1. Attends workshops and trainings related to the field
2. Ensures the safety of all individuals under his/her supervision and maintains a clean and organized and safe work area
3. Acts as a member of an agency committee
4. Facilitates off-hour drop-in center and/or activities as scheduled
5. Assists with lunch room duty and bus check-out as assigned

1. Ability to lift/push/pull up to 50 lbs.
2. Ability to see accurately both near and far
3. Ability to walk, bend, twist, kneel, and stoop
4. Ability to accurately manipulate small objects accurately using fine motor skills
5. Ability to drive to various locations as needed, adapting to various weather conditions

• Bachelor’s degree, Practical Nurse License or minimum of five years of supervised experience in mental health or human services
• One year experience with individuals having mental illness preferred
• Valid and acceptable driving record
• Excellent verbal and written skills
• Knowledge of computers required
