JOB SUMMARY: Develops and implements psychosocial rehabilitation services for individuals with mental illness for the purpose of recovery and community integration.
ESSENTIAL JOB FUNCTIONS:
- Coordinates intake procedures, completes paperwork and provides orientation to individuals entering programming
- Develops, coordinates, implements and monitors individual treatment plans and follow up for identified individuals on caseload
- Facilitates and coordinates community referral and linkage as necessary for individual consumers
- Maintains case files, updates medical exams, releases, etc. and ensures all reports (accident/illness, termination, discharge, etc.) are completed accurately and in a timely manner, in addition to attendance and benefits reports
- Assists PSR Coordinator in the monitoring of the Medicaid program, including: monthly billing, file audits, LPHA consultation, completing mental health assessments and ensuring Department of Human Services Rule 132 compliance
- Assists in the development and implementation of the Drop-In Center at NRI and LCHD, which is off site.
- Educate, promote and provide support to individuals in making healthy lifestyle choices, as referenced in the DSP health and safety and health and nutrition training curriculums and/or service plans.
- Identify and utilize effective advocacy strategies to overcome the diverse challenges facing individuals, i.e., human rights, legal, administrative and financial.
- Provides crisis intervention, behavior management and counseling as needed
- Responds to medical, mental health and behavioral emergencies as necessary
- Conducts and/or assists with psychiatric rehabilitation classes
- Assists with development of community resources/community integration
- Develops and implements the Workers Council
- Develop and implement PSR Volunteers program
- Attends and participates in Departmental meetings
- The employee may be asked to perform other duties as required by the employer’s business needs.
MARGINAL JOB FUNCTIONS:
- Attends workshops and trainings related to the field
- Ensures the safety of all individuals under his/her supervision and maintains a clean and organized and safe work area
- Acts as a member of an agency committee
- Facilitates off-hour drop-in center and/or activities as scheduled
- Assists with lunch room duty and bus check-out as assigned
PHYSICAL & VISUAL REQUIREMENTS:
- Ability to lift/push/pull up to 50 lbs.
- Ability to see accurately both near and far
- Ability to walk, bend, twist, kneel, and stoop
- Ability to accurately manipulate small objects accurately using fine motor skills
- Ability to drive to various locations as needed, adapting to various weather conditions
QUALIFICATIONS:
- Bachelor’s degree, Practical Nurse License or minimum of five years of supervised experience in mental health or human services
- One year experience with individuals having mental illness preferred
- Valid and acceptable driving record
- Excellent verbal and written skills
- Knowledge of computers required
