REPORTS TO:   Case Management Manager

JOB SUMMARY:   To provide case management services to individuals with developmental disabilities.
 
ESSENTIAL JOB FUNCTIONS:
1. Coordinates and conducts team meetings.  Develops service plans using a computer and ensures that plans are implemented properly and in a timely manner.
2. Responds to medical and/or behavioral crisis situations and provides support to fellow staff as needed.
3. Coordinates intake procedures, completes paperwork and provides orientation to individuals entering programming.
4. Maintains case files, updates medical exams, releases, etc.
5. Provides crisis intervention, behavior management and counseling as needed.  Responds to medical and/or behavioral emergencies.
6. Implements manual guidance techniques for individuals who exhibit self-abusive or aggressive behaviors.
7. Ensures that accident/illness, incident reports are documented and that information regarding the incident or accident is communicated to the appropriate people by the end of the day of the accident/illness incident.
8. Educate, promote and provide support to individuals in making healthy lifestyle choices, as referenced in the DSP health and safety and health and nutrition training curriculums and/or service plans.
9. Identify and utilize effective advocacy strategies to overcome the diverse challenges facing individuals, i.e., human rights, legal, administrative and financial.
10. Completes termination summaries and makes discharge recommendations.
11. Develops behavior modification programs as needed and presents to Behavior Management and Human Rights Committees.
12. The employee may be asked to perform other duties as required by the employer’s business needs.
 
MARGINAL JOB FUNCTIONS:
1. Assists individuals in accessing appropriate services and/or resources.
2. Serves as a liaison between the agency and the various other people who are involved in the individual’s care and/or life
3. Accompanies individuals on community outings as time allows.
4. May act as agency liaison for community meetings. 
 
PHYSICAL & VISUAL REQUIREMENTS:
1. Ability to lift/push/pull up to 50 lbs.
2. Ability to see accurately both near and far
3. Ability to walk, bend, twist, kneel, and stoop
4. Ability to accurately manipulate small objects accurately using fine motor skills
5. Ability to drive to various locations as needed, adapting to various weather conditions 
 
QUALIFICATIONS:
1. Bachelor’s degree in Human Services or related field.
2. Completed QSP training course
3. Certified as a QSP with the state of Illinois
4. One year of direct service related experience required.
5. Valid and acceptable driving record-No moving violations w/in the past 12 months.
6. Excellent verbal and written communication skills.
7. Knowledge of Microsoft Office required
